• Please allow 3 business days for routine refills. Compounded medications must be ordered and may take up to a week, so call in advance!
• This form is for  IN CLINIC PICKUP ONLY, not the online Pharmacy for home delivery.
• We will contact you if prescription information does not match previous refills or our doctors recommend a change in prescription.
• REMINDER: The pet receiving this prescription medication MUST have been examined within the last 12 months for routine refills.
